Job Summary: Provides direct and indirect patient care in the Emergency Department and Acute floor.  Provides care that meets the psychosocial, physical and general aspects of care; meets the communication needs of patient and family; provides care that reflects initiative and responsibility indicative of professional expectations, under the supervision of a registered nurse.  Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system.  Communicates with providers about changes in patient’s status, symptomatology and results of diagnostic studies.  Is able to respond quickly and accurately to changes in condition or response to treatment.

Major Responsibilities: (but not limited)

1. Provides direct patient care under supervision of a RN, evaluates outcomes, consults with other healthcare team members as required and adjusts nursing care processes as indicated to ensure optimal patient care.

2. Ability to perform a head-to-toe assessment on all patients, under the direction of an RN, and reassessments as per policy. This includes neonate, pediatric, adolescent and geriatric patients and the general patient population.

3. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.

4. Demonstrates knowledge of cardiac monitoring, identifies dysrhythmias.

5. Maintains current knowledge of medications and their correct administration based on age of the patient and his/her clinical condition.

6. Follows the seven (7) medication rights and reduces the potential for medication errors.

7. Demonstrates knowledge of lab requirements, requisitions.

8. Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; includes family in teaching, as appropriate.

9. Demonstrates knowledge of admission process to the hospital when necessary.

10. Maintains accurate and continued nursing documentation including patient histories, conditions, treatments, responses and assessment of changes.

11. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

12. Supports and maintains a culture of safety and quality.

13. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and family.

14. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patients served. Able to interpret data about the patient’s status in order to identify each patient’s age specific needs and provide care needed by the patient group.

15. Interacts professionally with patient and family and involves patient and family in the formation of the plan of care.

16. Maintains established hospital and department policies and procedures, objectives, performance improvement program, safety, environmental and infection prevention and control standards.

17. Maintains a safe, comfortable and therapeutic environment for patients and families in accordance with emergency department standards.

18. Ensures an adequate stock of supplies and proper functioning of equipment.

19. Assists in cost containment through appropriate ordering and conserving of supplies and equipment.

20. Maintains good working relationship with ED and Acute staff.

21. Demonstrates the ability to assist providers with procedures and perform services requiring technical and manual skills under the direction of an RN.

22. Medical and surgical asepsis is carried out during treatments and special procedures.

23. Demonstrates the ability to be flexible, organized and function under stressful situations.

24. Response to the patient is appropriate in emergency or physically distressful situations.

25. Manages and operates equipment safely and correctly.

26. Enhances professional growth and development through participation in educational programs, current literature, in-services and workshops; shows responsibility for ongoing education.

27. Performs other related duties as assigned within the LPN scope of practice.

Qualifications

: Graduate of a LPN nursing program State of Idaho licensed LPN One or more years LPN experience in Acute/ED preferred References indicating honesty, good attendance, dependability, performance and leadership abilities. English language skills adequate for effective communications with staff, patients, medical and allied support personnel, public Positive supportive/coaching approach to co-workers. Occupational Therapy Assistants are responsible for treating patients with therapeutic and self-care activities. The job requires candidates to perform a variety of duties including the following:

• Monitoring a patient’s progress to make sure that they’re successfully completing treatment

• Encouraging the patient during their recovery process

• Helping patients to improve their quality of life by finding solutions to their problems

• Helping injured employees improve their motor skills with the goal of re-entering the workforce

• Teaching patients with disabilities to become more independent

• Updating patient records for the Occupational Therapist

• Keeping the office, treatment room and equipment in good order

• Teaching patients, family members or other Caregivers skills and techniques to help with the patients’ treatment program

Education

Accreditation Council for Occupational Therapy Education (ACOTE) recognized associate degree. Must have passed the Certified Occupational Therapy Assistant exam.

To provide Restorative Nursing care to the residents of the facility safely and keeping in mind the goal of maintaining or improving their functional mobility.  Perform Safe Ambulation, Transfers and Range of Motion (ROM), assist with activities of daily living and perform other related duties as assigned by supervisor.

Duties and Responsibilities:

Work under the direction of the Restorative Nurse/Director of Nursing

 Work with restorative nurse to assess ADL abilities of residents newly admitted to facility, recently discharged from therapy, and with every MDS assessment.

 Collaborate with RN to create a care plan for each resident in the restorative program. 

 Collaborate with RN to revise and update care plans with any change in the resident’s abilities and with every restorative assessment.

 Attend all care plan meetings.

 Work closely with Therapy department by:

• Following programs as prescribed

• Consulting with Therapy department as needed

•  Instruct nursing assistants in proper procedures.

Assist and instruct residents with daily ROM exercises and restorative exercises as outlined in each resident’s personalized care plan.

 Chart restorative cares in flow sheet and add notes for any exceptions, special circumstances, or additional information in restorative notes.

 Collaborate with restorative nurse to evaluate the restorative potential of every resident who has a fall.

 Attend facility meetings

 Assist in getting residents dressed and ready for the day before breakfast and helping residents to and from meals as time allows when not doing restorative exercises with other residents

Education/Training:

Equivalent of Completion of High School

PERFORMANCE STANDARDS‍ ‍

1. Maintain compliance regarding patient and resident rights.

2. Perform all work in accordance with all federal, state, and facility regulations.

3. Monitor and wash dishes as they accumulate to ensure clean work environment. This includes dishes in the cafeteria.

4. Clean and maintain dishwasher area as needed following facility and manufacturer instructions.

5. Maintain record of dishwasher temperatures. Inform supervisor if temperatures are out of acceptable range

6. Maintain and stock the cafeteria and kitchen to include drawers, fridge, snacks, also stock the kitchen as needed with containers, napkins, supplements, and whatever else is needed.

7. Monitor and replace all cleaning solutions used by the dishwasher and 3 compartments sink as needed.

8. Follow daily menus to prepare trays with diet cards, beverages, supplements, fruit, desserts as needed.

9. Transport meals via meal cart to Hospital and LTC.

10. Report broken or damaged utensils and any malfunctions in equipment to supervisor.

11. Record all food and supplement temps on temp sheet before tray line.

12. Prepares and plates quality food accurately and attractively according to diet and menu while adhering to nutritional needs of the patient/resident.

13. Provide customer service to patients, residents, and patrons while maintaining high standards of quality, sanitation and safety.

14. Ensure proper sanitation and storage of kitchen equipment, food and supplies.

15. Prepare for upcoming meals by allowing appropriate thawing according to HACCP.

16. Maintain kitchen sanitation and safety standards. 

17. Chart temperatures of refrigerators and freezers, total number of meals served daily (including guest trays), and completion of tasks.

18. Use first-in, first-out, method of food usage.

19. Assist in the cleaning of the kitchen, kitchen equipment, food storage, and supply areas according to the facility schedule.

20. Dispose of all garbage per sanitation regulations

21. Adhere to the established dietary standards for personal hygiene and personal appearance. See employee manual.

Full-Time | Business Office

Purpose:

This position is a critical part of the Revenue Cycle teams. Responsibilities cover a wide range of interesting duties and has a great deal of interaction with other Power County Hospital District (PCHD) Staff. This position directly supports the Business Office Manager, credentials providers and facilities with payers and performs other revenue cycle duties. This position does not directly supervise other employees. Travel is limited.

Responsibilities:

·         Support the functions of the Business Office by serving as the assistant to the Business Office Manager.

o   Provide complete and accurate information to payers to facilitate correct listings on their websites.

o   Periodically review insurance company directories to ensure our providers and facilities are all listed and the information accurate and up to date. Work with insurance companies to remove inactive providers and fix incorrect information. 

o   Periodically review CMS and Medicaid websites (NPPES NPI registry, PECOS, etc.) to ensure our providers and facilities are all listed and the information accurate and up to date. Work with the appropriate agencies to remove inactive providers and fix incorrect information. 

o   Duties and special projects as assigned by Business Office Manager or CFO.

·         Payer Credentialing

o   Accurately complete and timely submit credentialing (and re-credentialing) applications for both providers and facilities.  This includes credentialing for Medicare, Medicaid, and various commercial and other insurance plans

o   Initiate and maintain CAQH (Council for Affordable Quality Healthcare) files on all Providers

o   Maintain files with provider and facility licenses, malpractice insurance and other supporting documentation.

o   Maintain copies of all credentialing applications and important correspondence.

o   Track and manage credentialing dates to ensure re-credentialing is started and completed in a timely fashion.

o   Actively seek out information on insurance plans being offered in our area (both employer and healthcare exchange) and work to complete credentialing for them.

·         Revenue Cycle Assurance

o   Periodically audit payments from Medicare, Medicare Advantage and commercial plans to ensure we are being paid according to the most recent contracted rates.

o   Work with CFO, Patient Accounting supervisor and payer representatives to address and resolve any payment/reimbursement issues.

o   Work with Patient Accounting Supervisor to resolve and dispute payment denials

o   Work with Revenue Cycle team to help drive improvements in the charge capture process.

o   Distribute the most current Medicare rate letters to Medicare Advantage plans.

o   Provide support for HIM including assisting with medical records requests, scanning and other projects.

Purpose

The Healthcare IT Systems Administrator is responsible for the administration, maintenance, and support of the organization's information technology systems and infrastructure. This role supports a hybrid environment including Active Directory and Microsoft Entra ID, Microsoft 365, Azure Virtual Desktop, endpoint and network infrastructure, clinical and business applications, and end-user technology.

The position provides technical support to physicians, nurses, and clinical and administrative staff while assisting with the implementation, maintenance, and improvement of technology solutions across the organization.

Responsibilities

Systems & Hybrid Identity Administration

• Administer and support Active Directory, Microsoft Entra ID, Microsoft 365, and associated hybrid identity services.

• Manage user accounts, security groups, permissions, licensing, and access throughout the employee lifecycle, including onboarding, role changes, and offboarding.

• Monitor and troubleshoot Entra ID synchronization, authentication, MFA enrollment, and account access issues.

• Configure and maintain Windows servers and core services, including Group Policy, DNS, DHCP, and file services.

• Apply least-privilege principles to account and permission changes, and support periodic access reviews.

Azure Virtual Desktop & Cloud Infrastructure

• Administer the organization's Azure Virtual Desktop (AVD) environment, including host pools, session hosts, application delivery, and user access.

• Use Nerdio for provisioning, configuration, scaling, and ongoing management of virtual desktop resources.

PCHD is looking for a Speech Language Pathologist to provide services for skilled nursing patients, acute/swing bed patients and any community members in need of speech therapy. This person will have the opportunity to develop and grow the position requiring a self starter, out-going, personable, skilled therapist.
