STARS & STRIDES 5K PARTICIPANT WAIVER, RELEASE OF LIABILITY, AND MEDIA CONSENT

By registering for and participating in the Stars & Strides 5K, I acknowledge and agree to the following:

I understand that participation in this event involves physical activity and outdoor community event activities, including walking, exposure to weather conditions, uneven surfaces, sidewalk chalk activities, vendor areas, health and wellness stations, and other event-related activities.

I voluntarily assume all risks associated with participation, including but not limited to slips, falls, allergic reactions, weather-related conditions, contact with other participants, and other risks that may arise from participation in a public community event.

I certify that I am physically able to participate in this event and have not been advised otherwise by a qualified healthcare provider.

In consideration of being permitted to participate, I release, waive, discharge, and hold harmless the Power County Hospital District Foundation, Power County Hospital District, the City of American Falls, event organizers, volunteers, vendors, sponsors, employees, agents, and representatives from any and all claims, liabilities, damages, losses, or expenses arising out of or related to my participation in the event.

I understand that emergency medical treatment may be provided if deemed necessary during the event and accept responsibility for any resulting medical expenses.

I grant permission to the Power County Hospital District Foundation and its representatives to photograph, videotape, and record my participation in the event and to use such images or recordings for promotional, educational, marketing, fundraising, and public relations purposes without compensation.

If registering a minor child, I certify that I am the parent or legal guardian of the child and consent to the child's participation under the terms of this agreement. I accept responsibility for the child's participation and release the above parties from claims arising from the child's participation.

By checking the acknowledgment box and completing registration, I confirm that I have read, understand, and voluntarily agree to the terms of this Waiver, Release of Liability, and Media Consent Agreement.
