Sports Physicals

Before participating in school or recreational athletics, many students are required to complete a sports physical. Our providers perform comprehensive evaluations to help ensure athletes are healthy and ready for participation.

What Is a Sports Physical?

A sports physical evaluates your child's overall health and helps identify any medical conditions that could affect safe participation in sports and other physical activities.

Your provider will complete the required forms and discuss any recommendations before participation.

Please Bring

  • Sports physical forms (if provided by the school)

  • Current medication list

  • Glasses or contact lenses (if worn)

  • Insurance card

  • Parent or guardian (when required)
